•All clinics are sponsored by Elk Grove Village and
Alexian Brothers Hospital Network.
• Flu shots are $20.00 (cash or personal checks only, no credit cards). Anyone with Medicare must bring their Medicare card to the clinic (no HMO, PPO or third party payers will be accepted).
• Elk Grove Village Residents Only.
• Vaccine will not be administered to anyone under 19 at these clinics. Children's Flu clinic cancelled.
